Registration

To register please fill out all proper forms and mail to:

If registering on-line please open the Registration Form to print off Medical and Consent forms. These must be mailed to the above address along with a cheque or money order.

The cost to attend camp is $250. Each registration form must be accompanied by a $100 non-refundable deposit due April 1, 2010. Payment of $150 for remaining balance will be due by June 7, 2010. Please make cheques or money orders payable to "Ultimate Hockey Camp". Your Child's registration is confirmed ONLY after the deposit is received. Space in all camps is limited and is strictly on a first come, first serve basis.
